Insurance & Payment Information
What is a Superbill?
Please Note
At this time, Zap House is an out-of-network provider and payment is due at the time of service. We are actively pursuing insurance credentialing and will share updates as they become available.
For clients seeking reimbursement through their insurance plan, we can provide an insurance-coded invoice, often referred to as a superbill, upon request. A superbill contains the information insurance companies typically require when reviewing out-of-network reimbursement claims.
If you are interested in using insurance once we become credentialed, you can join our insurance waitlist by completing the form below. We will contact you when we begin accepting your insurance plan or have updates regarding our credentialing status.
A superbill is an itemized receipt that includes treatment information, provider details, diagnosis codes, and billing codes that may be required by your insurance company for reimbursement consideration.
Because coverage and reimbursement policies vary by plan, we recommend contacting your insurance company directly to determine:
Whether electrolysis services are covered
Whether out-of-network reimbursement is available
Any documentation requirements
Applicable deductibles, copays, or reimbursement limits
Payment is due at the time of service.
Reimbursement is not guaranteed and is determined solely by your insurance carrier.
Coverage requirements vary by plan and diagnosis.
Clients are responsible for submitting superbills to their insurance company unless otherwise instructed by their insurer.
Zap House cannot guarantee reimbursement amounts or claim approval.
